Printing eAYSO Forms

While logged in to eAYSO with the same account where you entered the information, follow these instructions.

1. Press the parents button
2. Press the Click Here button next to " Print my form or print another copy of a previously printed form"
3. Press the printer icon next to the name of the person whose form you want to print.
4. Click the "Click here to print" link at the top of the page.
5. Print two copies for each player or volunteer and sign both. Since the forms are two pages, please print on both sides of sheet, so you only use one sheet. Make sure any browser pop-up blockers are off. You will not be able to print the forms with a pop-up blocker on.
