Printing eAYSO Forms

While logged in to eAYSO with the same account where you entered the information, follow these instructions.

1. Select "Print player form" or "Print form".
2. Select the player's or volunteer's name
3. Print one copy for each player and two copies of each volunteer form and sign and initial the forms in the appropriate places. Please print on one side of the paper, in portrait (not landscape) orientation.
