Player Registration Instructions
- Consider how you can volunteer to help our region succeed. We need a coach, assistant coach, referee, assistant referee, and team parent for every team. Our volunteers needed page shows other volunteer positions we need to fill.
- Fill out the player registration forms on-line at eayso.org.

Instructions
for those who have not used eAYSO before.
- Print
and sign two copies of the registration forms for each player. Don't simply print using your browser print button, but select the print options within eAYSO, and make sure you get the pre-registration number to appear on your form. Some people find printing the forms difficult, so we have created instructions for printing eAYSO forms.
- Print and fill out our Volunteer Form (pdf) or the eAYSO volunteer form.
-
Bring all copies and a check for $110 per player to late registration

Volunteer Registration Instructions
Because we need to protect our children, we require most volunteers to officially register in the eAYSO database. Following are the steps to do that.
- Log in to eAYSO with the same account you used to register the player.
- Press the volunteers button
- Choose the appropriate Click Here button next to one of the following
- Sign up as a NEW Volunteer to help in your region
- Sign up as a new Youth Volunteer (ages 12-17)
- Returning volunteer(s): ALL Volunteers need to re-register each year
- Follow the instructions for entering all the information
- Print two copies of the volunteer form. If you have trouble printing the forms, look at the instructions for printing eAYSO forms.
- Mail (don't e-mail) the two signed copies of the volunteer form to our Child and Volunteer Protection Advocate (contact information is at Contact Us).
Why We Don't Register Players by Mail / Web
There are several reasons:
- We cannot effectively recruit volunteers through the mail / web, and if we cannot do this, AYSO will cease to exist.
- We cannot check to be sure all forms or payments are complete and correct before you mail them, and the cost of communication to resolve such problems is too high.
- Processing registration is inefficient and error-prone for stacks of mail as opposed to forms processed at registration.
-
We must collect signed emergency authorizations for our coaches to be able to get medical treatment for an injured player. It is illegal for a doctor to treat a minor without a signed authorization from the parent unless it is a life or limb threatening emergency. The doctor could be prosecuted for assault and battery if they treat a minor without consent.
Refunds
If you want to drop out of playing, see our refund policy.
